Masonic Name Badge Order Form

Brethren,

If you are interested in having a name badge made, please fill out the form below. All badge orders will be placed for production once paid in full by the member. Badges will only be made for Master Masons in good standing at a cost of $17.50 each. It is expected that once an order has been placed here that the member present or mail a check made to Lombard Lodge No. 1098 AF&AM prior to the next Stated meeting. In memo field of check please note "Badge" for proper accounting.
